การออกใบแทนใบอนุญาต ให้ผู้อำนวยการใช้แบบใบอนุญาตเดิมในสาขาที่ได้ ขึ้นทะเบียนและรับใบอนุญาต และระบุคำว่า “ใบแทน” ด้วยตัวอักษรสีแดงไว้ในวงเล็บต่อท้ายเลขที่ ใบอนุญาต
