2 sectionsการยื่นคำขอ การแจ้ง หรือการติดต่อใด ๆ และการออกเอกสารหลักฐานต่าง ๆ รวมทั้งใบอนุญาตและใบแทนใบอนุญาต ตามกฎกระทรวงนี้ ให้ดำเนินการตามกฎหมายว่าด้วย การปฏิบัติราชการทางอิเล็กทรอนิกส์ ในกรณีที่มีเหตุไม่สามารถดำเนินการโดยวิธีการทางอิเล็กทรอนิกส์ได้ เล่ม ๑๔๒ ตอนที่ ๒๙ ก ให้ดำเนินการ ณ สำนักงาน อย. กระทรวงสาธารณสุข หรือสถานที่อื่นตามที่เลขาธิการ อย. กำหนด โดยประกาศในราชกิจจานุเบกษา ในกรณีที่เป็นการยื่นคำขอ การแจ้ง หรือการติดต่อใด ๆ ตามข้อ ๖ (๔)
(ก) อาจดำเนินการ ณ สำนักงานสาธารณสุขจังหวัดแห่งท้องที่ที่สถานที่ทำการของผู้ขออนุญาตนั้นตั้งอยู่ก็ได้
คำขอ ใบอนุญาต และใบแทนใบอนุญาตตามกฎกระทรวงนี้ ให้เป็นไปตามแบบที่ เลขาธิการ อย. กำหนดโดยประกาศในราชกิจจานุเบกษา
