บุคคลซึ่งประสงค์จะขอขึ้นทะเบียนเป็นพนักงานตรวจโรคสัตว์ให้ยื่นคำขอต่อนายทะเบียน ตามแบบที่อธิบดีประกาศกำหนด พร้อมด้วยข้อมูล เอกสาร หรือหลักฐานที่แสดงว่ามีคุณสมบัติ ตามข้อ ๒
