ในกรณีที่ใบอนุญาตชำรุดในสาระสำคัญ สูญหาย หรือถูกทำลาย ให้ผู้รับใบอนุญาต ยื่นคำขอรับใบแทนใบอนุญาตต่อเลขาธิการ พร้อมด้วยเอกสารหรือหลักฐานตามที่ระบุไว้ในแบบคำขอรับ ใบแทนใบอนุญาต ภายในสิบห้าวันนับแต่วันที่ได้รับทราบถึงการชำรุดในสาระสำคัญ สูญหาย หรือ ถูกทำลาย ให้นำความในข้อ ๖ และข้อ ๘ มาใช้บังคับแก่การพิจารณาคำขอรับใบแทนใบอนุญาต โดยอนุโลม ใบแทนใบอนุญาตให้ใช้แบบใบอนุญาต โดยระบุคำว่า “ใบแทน” ด้วยอักษรสีแดงไว้ด้านบน ของใบอนุญาต
