ผู้รับใบอนุญาตที่ประสงค์จะแก้ไขเพิ่มเติมชื่อตัว ชื่อสกุล หรือชื่อตัวและชื่อสกุล หรือรายการประวัติอื่นใดในใบอนุญาต ให้ยื่นคำขอแก้ไขเปลี่ยนแปลงรายการดังกล่าวต่อเลขาธิการ พร้อมด้วยเอกสารหรือหลักฐานตามที่ระบุไว้ในแบบคำขอแก้ไขเปลี่ยนแปลงรายการในใบอนุญาต ให้นำความในข้อ ๖ และข้อ ๘ มาใช้บังคับแก่การพิจารณาคำขอแก้ไขเปลี่ยนแปลงรายการ ในใบอนุญาตโดยอนุโลม
