ใบรับคำขอขึ้นทะเบียน ใบสำคัญ ใบรับคำขออนุญาต ใบอนุญาต ใบแทนใบสำคัญ ใบแทนใบอนุญาต และคำขอตามกฎกระทรวงนี้ ให้เป็นไปตามแบบที่อธิบดีกำหนด แบบตามวรรคหนึ่ง อธิบดีจะกำหนดให้แตกต่างกันตามประเภทของงานก็ได้
