ผู้รับอนุญาตนำหรือสั่งยาแผนโบราณเข้ามาในราชอาณาจักรที่ประสงค์จะนำหรือสั่งยา เข้ามาเป็นตัวอย่างเพื่อขอขึ้นทะเบียนตำรับยา ให้ยื่นคำขอตามแบบ ย.บ.๘ ท้ายกฎกระทรวงนี้ จำนวนสองชุด
