เมื่อพนักงานเจ้าหน้าที่ได้รับแบบรายการที่นายจ้างได้ยื่นต่อสำนักงานตามมาตรา ๓๔แล้ว ให้ตรวจสอบความถูกต้องและเสนอต่อเลขาธิการหรือผู้ซึ่งเลขาธิการมอบหมายเพื่อออกหนังสือสำคัญ แสดงการขึ้นทะเบียนประกันสังคมให้แก่นายจ้างและออกบัตรประกันสังคมให้แก่ลูกจ้าง
