2 sectionsการยื่นคำขอ การแจ้ง หรือการติดต่อใด ๆ และการออกเอกสารหลักฐานต่างๆ รวมทั้งใบอนุญาตและใบแทนใบอนุญาต และการชำระค่าธรรมเนียมตามกฎกระทรวงนี้ ให้ดำเนินการ ตามกฎหมายว่าด้วยการปฏิบัติราชการทางอิเล็กทรอนิกส์ ในกรณีที่มีเหตุไม่สามารถดำเนินการ โดยวิธีการทางอิเล็กทรอนิกส์ได้ ให้ดำเนินการ ณ สำนักงาน อย. กระทรวงสาธารณสุข หรือสำนักงานสาธารณสุขจังหวัดแห่งท้องที่ที่สถานที่ทำการของผู้ขออนุญาตนั้นตั้งอยู่ หรือสถานที่อื่น ตามที่เลขาธิการ อย. กำหนดโดยประกาศในราชกิจจานุเบกษา
คำขอ ใบอนุญาต และใบแทนใบอนุญาตตามกฎกระทรวงนี้ ให้เป็นไปตามแบบ ที่เลขาธิการ อย. กำหนดโดยประกาศในราชกิจจานุเบกษา
