ในกรณีที่ใบสำคัญการขึ้นทะเบียนวัตถุตำรับสูญหายหรือถูกทำลายในสาระสำคัญ ให้ผู้รับอนุญาตยื่นคำขอรับใบแทนใบสำคัญการขึ้นทะเบียนวัตถุตำรับต่อผู้อนุญาตภายในสิบห้าวันนับแต่ วันที่ได้ทราบถึงการสูญหายหรือถูกทำลายดังกล่าว การขอรับใบแทนใบสำคัญการขึ้นทะเบียนวัตถุตำรับและการออกใบแทนใบสำคัญการขึ้นทะเบียน วัตถุตำรับ ให้เป็นไปตามหลักเกณฑ์ วิธีการและเงื่อนไขที่กำหนดในกฎกระทรวง
