การอุทธรณ์ต้องทำเป็นหนังสือลงลายมือชื่อผู้อุทธรณ์ ในหนังสืออุทธรณ์ตามวรรคหนึ่ง ต้องใช้ถ้อยคำสุภาพ และระบุข้อเท็จจริงและเหตุผลอันเป็นเหตุ แห่งการอุทธรณ์ให้ชัดเจน พร้อมแนบเอกสารหลักฐานที่เกี่ยวข้องไปด้วย ในกรณีที่เห็นสมควร รัฐมนตรีอาจออกระเบียบกำหนดวิธีการอุทธรณ์เป็นอย่างอื่นหรือรายละเอียด เกี่ยวกับการอุทธรณ์อื่นด้วยก็ได้
