ผู้ใดประสงค์จะมีไว้ในครอบครองหรือใช้วัสดุนิวเคลียร์ ประเภทที่ ๑ ประเภทที่ ๒ หรือประเภทที่ ๓ แล้วแต่กรณี ให้ยื่นคำขอรับใบอนุญาตมีไว้ในครอบครองหรือใช้วัสดุนิวเคลียร์ต่อ เลขาธิการ พร้อมด้วยเอกสารหรือหลักฐานตามที่ระบุไว้ในแบบคำขอรับใบอนุญาต
