ผู้ใดประสงค์จะทำเครื่องกำเนิดรังสี ให้ยื่นคำขอรับใบอนุญาตต่อเลขาธิการ พร้อมด้วยเอกสารหรือหลักฐานตามที่ระบุไว้ในแบบคำขอรับใบอนุญาต การขอรับใบอนุญาตตามวรรคหนึ่ง ผู้ขอรับใบอนุญาตต้องยื่นหลักฐานแสดงวิธีการทำ เครื่องกำเนิดรังสี และวิธีการตรวจสอบคุณภาพและความปลอดภัยของเครื่องกำเนิดรังสีตามที่เลขาธิการ ประกาศกำหนดด้วย เลม่ ๑๓๘ ตอนที่ ๖๖ ก หนา้ ๑๒
