ให้ผู้ขออนุญาตดำเนินการขออนุญาตปฏิบัติการอำพรางโดยทำเป็นหนังสือต่อผู้มี อำนาจอนุญาต และระบุเหตุผล ความจำเป็น และแผนการหรือวิธีการ รวมทั้งระยะเวลาในการ ดำเนินการและรายละเอียดอื่น ๆ ที่เกี่ยวข้อง เล่ม ๑๒๙ ตอนที่ ๓๕ ก หนังสือขออนุญาตตามวรรคหนึ่ง ให้เป็นไปตามแบบที่เลขาธิการคณะกรรมการป้องกัน และปราบปรามยาเสพติดกำหนด
