ผู้ใดประสงค์จะขออนุญาตผลิต ขาย นำหรือสั่งเข้ามาในราชอาณาจักร ซึ่งยาแผนโบราณ ให้ยื่นคำขอพร้อมด้วยเอกสารและหลักฐานตามที่ระบุไว้ในแบบ ย.บ.๑ ท้ายกฎกระทรวงนี้
