เมื่อหญิงที่รับตั้งครรภ์แทนจะไปฝากครรภ์หรือไปคลอดบุตรยังสถานพยาบาลใด ให้นำข้อตกลงตามมาตรา ๒๕ไปแสดงต่อแพทย์ผู้รับฝากครรภ์หรือผู้ที่จะทำคลอด ณสถานพยาบาลแห่งนั้น เพื่อเป็นหลักฐานในการออกหนังสือรับรองการเกิดและการแจ้งการเกิดของเด็กต่อไป
