ผู้ใดประสงค์จะให้กรมพัฒนาที่ดินทำการปรับปรุงดินหรือที่ดิน หรือการอนุรักษ์ดิน และน้ำเป็นการเฉพาะราย ให้ยื่นคำขอตามแบบที่คณะกรรมการกำหนด ณ กรมพัฒนาที่ดินสำนักงาน พัฒนาที่ดินเขต หรือสถานีพัฒนาที่ดินท้องที่ซึ่งที่ดินนั้นตั้งอยู่ พร้อมด้วยเอกสารและหลักฐานตามที่ระบุ ไว้ในแบบคำขอตามที่คณะกรรมการกำหนด
