ผู้รับอนุญาตเกี่ยวกับยาแผนโบราณทุกประเภทที่ประสงค์จะย้ายสถานที่จากสถานที่ ซึ่งระบุไว้ในใบอนุญาต ให้ยื่นคำขอพร้อมด้วยเอกสารและหลักฐานตามที่ระบุไว้ในแบบ ย.บ.๑๔ ท้ายกฎกระทรวงนี้ การอนุญาตให้ย้ายสถานที่ตามวรรคหนึ่งให้กระทำโดยวิธีสลักหลังใบอนุญาต หรือออกหนังสือ แนบท้ายใบอนุญาต
