ผู้รับอนุญาตเกี่ยวกับยาแผนโบราณทุกประเภทที่ประสงค์จะขอต่ออายุใบอนุญาต ให้ยื่นคำขอตามแบบ ย.บ.๑๓ ท้ายกฎกระทรวงนี้ การอนุญาตให้ต่ออายุใบอนุญาต จะแสดงไว้ในรายการท้ายใบอนุญาตนั้น หรือจะออกใบอนุญาต ให้ใหม่ก็ได้
