ในกรณีที่ใบอนุญาตสูญหายหรือถูกทำลาย ให้ผู้รับใบอนุญาตยื่นคำขอรับใบแทน ใบอนุญาตต่อผู้อนุญาตภายในสามสิบวันนับแต่วันที่ได้ทราบถึงการสูญหายหรือถูกทำลายพร้อมด้วย ข้อมูล เอกสาร หรือหลักฐานตามที่กำหนดในแบบคำขอรับใบแทนใบอนุญาต ให้นำความในข้อ ๓ ข้อ ๔ วรรคหนึ่ง และข้อ ๕ มาใช้บังคับแก่การพิจารณาคำขอรับ ใบแทนใบอนุญาตและการออกใบแทนใบอนุญาตด้วยโดยอนุโลม
